The forms below are used to register for Religious Education classes.  The completed form(s) can be mailed to the Religious Education Office at the address above, dropped off at the Religious Education Office, or placed in the Sunday collection basket.

NON-PARISHIONER REGISTRATION FORM:

If you are not registered at St. Leonard Parish, but would like to enroll your child(ren) in our Religious Education program click on the link below and print a copy of the form.  The completed from can be mailed to the address above.

To obtain a copy of the Photo Consent form, click on the link below.  This form allows St. Leonard to publish pictures of your child(ren) on our website or in our parish/religious education newsletters or other publications.
